Uniting Communities is seeking self-motivated people to join our new Aged Care Assessment (ACAS) team!
About The Role
Clinical Home Support Assessors leverage their clinical expertise to conduct thorough assessments for individuals with complex needs seeking support through the My Aged Care portal. They collaborate daily with clients, carers, and service providers to facilitate access to appropriate support service
You will report to the Team Leader but are expected to work independently and effectively within the community.
The role responsibilities include (but not limited to)

Undertake comprehensive and home support assessments for clients via the My Aged Care Portal using the Integrated Assessment Tool (IAT).

Conduct in-depth evaluations of clients' medical conditions, cognitive function, mobility, and psychosocial needs.

Assess clients' informal support networks and determine their long-term support requirements.

Offer short-term case management services when required.

Utilise online assessment tools and be proficient in the use of web-based portal

About You
To be successful in this role, you need to demonstrate the ability to assess long-term support options and determine eligibility for comprehensive aged care services, including residential and home care packages.
You also will need the following

Tertiary qualifications in a health-related discipline directly related to health, aged care, or related area

Current AHPRA Registration or eligibility for AASW Membership

Current Australian driver’s license and comfortable driving in the community

Valid National Criminal History Check and Vulnerable Persons Check

Living close to region required to work within.
