We are seeking dedicated and compassionate Part Time and Full time Registered Nurses to join our community aged care team. In this role, you will work in the community supporting clients in their homes. You will work collaboratively within a multidisciplinary team to deliver high-quality nursing care and support client’s overall well-being.

Reporting to the Clinical Manager, you will be providing high level support and assistance to our community clients, ensuring their right to choice, independence and dignity.

Key duties and responsibilities will include:

  • Providing holistic nursing care to clients, ensuring their individual needs are met with dignity and respect.
  • Complete comprehensive nursing assessments to aid in the development of individualised care plans for our clients.
  • Collaborate effectively with care partners, allied health professionals, and families to develop and implement personalized care plans.
  • Maintain accurate and timely documentation in line with organisational policies and procedures.
  • Contribute to continuous improvement initiatives to enhance the quality of care and services provided

About You:

The successful candidate will have:

  • Passion for working with others in a community aged care setting.
  • Current Registered Nurse registration with AHPRA.
  • Current flu vaccination certificates.
  • Current National Criminal History Checks or the willingness to get one.
  • Ability to communicate effectively with clients, families, and all levels of staff.
  • Current drivers license and reliable vehicle
  • Experience in a community aged care setting is desirable.
