Are you a health professional looking to use your clinical expertise in a role where your judgement can make a genuine difference?  This is fully remote role!

Uniting Communities is looking for a Triage Assessor to join our Aged Care Assessment Service (ACAS) team. This is an opportunity to move beyond traditional clinical practice and use your experience to help older Australians access the right support at the right time.

As a Triage Assessor, you will undertake phone-based evaluations to understand a client's health, cognitive, mobility and psychosocial needs, as well as their informal supports. You will use your clinical judgement to determine eligibility for support under the Aged Care Act, identify the most appropriate assessment pathway and allocate clients to an assessor with the right experience and credentials.

You will also play an important role in identifying and responding to urgent referrals, ensuring clients receive appropriate support within required timeframes.

What you'll do

  • Conduct phone-based evaluations of clients' needs and circumstances
  • Use clinical judgement to determine eligibility and appropriate pathways
  • Assess medical, cognitive, mobility and psychosocial needs
  • Consider informal supports and short- and long-term support requirements
  • Identify and refer urgent matters appropriately
  • Use My Aged Care and online triage and assessment tools
  • Allocate clients to assessors based on their needs and credentials
  • Build effective relationships with internal and external stakeholders
  • Maintain accurate and timely client records
  • Apply your knowledge of funded and non-funded aged care services
  • Work within the ACAS Service Framework and relevant legislation

The role is designed for someone who can work independently, manage competing priorities and make sound decisions while maintaining a strong focus on client outcomes.

About you

You will bring a relevant tertiary qualification in a health-related discipline, such as:

  • Registered Nursing
  • Occupational Therapy
  • Physiotherapy
  • Social Work

You will need current AHPRA registration or eligibility for AASW membership, as applicable.

You will also need to hold, or be willing to undertake, the Statement of Attainment – Home Support Assessors (SOA2) MAC Learning Certification, together with required training modules and ongoing assessor approval. Experience in aged care assessment is preferred.

  • Knowledge of the aged care and community care sectors
  • An understanding of the physical and psychological aspects of ageing
  • Strong communication and relationship-building skills
  • The ability to manage competing priorities and multitask
  • Strong administrative and organisational skills
  • Experience using computer-based assessment or client data systems
  • A commitment to culturally respectful practice with Aboriginal and Torres Strait Islander people
  • A strong and ethical approach to customer service
