Enrolled Nurse or Registered Nurse (Community Aged Care)

We are seeking dedicated and compassionate Enrolled Nurses (ENs) and Registered Nurses (RNs) to join our community aged care team on a permanent part-time basis. In this role, you will work in the community supporting clients in their homes. You will collaborate with a multidisciplinary team to deliver high-quality nursing care and support clients' overall wellbeing.
Reporting to the Clinical Manager, you will provide high-level support and assistance to our community clients, ensuring their right to choice, independence, and dignity.
Key duties and responsibilities will include:
- Providing holistic nursing care to clients, ensuring their individual needs are met with dignity and respect.

Complete comprehensive nursing assessments to aid in the development of individualised care plans for our clients.

Monitor clients' health status and identify changes or emerging clinical needs.
- Collaborate effectively with care partners, allied health professionals, and families to develop and implement personalized care plans.

Provide education and support to clients and their families regarding health management and available services.
- Maintain accurate and timely documentation in line with organisational policies and procedures.
- Contribute to continuous improvement initiatives to enhance the quality of care and services provided
The successful candidate will have:
- Passion for working with others in a community aged care setting.
- Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
- Current flu vaccination certificate.
- Current National Criminal History Check or willingness to obtain one.
- Ability to communicate effectively with clients, families, and all levels of staff.
- Current driver's licence and reliable vehicle.
- Experience in a community aged care setting is desirable.
