We are seeking a passionate and experienced Coordinator – Community Connections to join our Community Services team.
In this role, you will support vulnerable and socially isolated adults aged 18–64 who are not eligible for NDIS or My Aged Care, helping them access the supports they need to improve wellbeing, independence, and community participation.
This is a dynamic and rewarding position where you will lead coordination, case management, and service delivery across multiple programs and stakeholders.
About the role
As a Coordinator, you will be responsible for delivering and overseeing short to medium-term, person-centred support services. You will work collaboratively with internal teams, community partners, and external stakeholders to ensure high-quality outcomes for participants.
Key responsibilities include:
- Coordinating and case managing individual support plans
- Conducting assessments, risk assessments, and ongoing reviews
- Developing and implementing Individual Connection Plans
- Ensuring timely and accurate documentation in case management systems (CSnet / R2D2)
- Responding to referrals and coordinating service delivery
- Working closely with community partners and stakeholders
- Supporting program delivery, reporting, and KPI requirements
- Contributing to service development and continuous improvement
- Providing coordination support to the Service Manager as required
- Participating in community engagement and stakeholder networks
About you
You are a compassionate, organised, and solutions-focused professional who thrives in a busy and evolving environment. You are confident managing competing priorities while maintaining a strong commitment to person-centred practice.
You will bring:
- Tertiary qualification in a relevant field (required)
- Experience working with people with mental health and/or complex needs
- Experience in case management or community services
- Strong understanding of person-centred and strengths-based approaches
- Excellent written and verbal communication skills
- Ability to work independently and manage multiple priorities
- Strong collaboration and stakeholder engagement skills
- Commitment to inclusive and culturally safe practice
Highly regarded
- Experience in team coordination, or mentoring
- Knowledge of the NDIS and broader sector reforms
- Experience using case management systems (e.g. CSnet)
- Experience working with family carers and support networks
Requirements
- Current unrestricted Driver’s Licence
- First Aid Certificate
- National Police Check
- Working With Children Check
